your medical record and your results

During your hospital stay, you may request to review your medical file. When you do, a member of the healthcare team will accompany you to help explain medical terms and answer any questions you may have.

You may also contribute directly to your medical record by using the “Patient and Care Partner Comment Form.” The healthcare team will review your comments and respond to any care-related questions you include.

Access to electronic medical information

You can also access your results (such as lab and radiology) online. You will be provided with a username and password. If you encounter any difficulties accessing your results, our team is available to assist you.

Requesting a copy of your file

You may request a copy of your file from the Medical Records department after your discharge by presenting personally to our Medical Records Department with your identity card.

If you are not able to come and want the legal representative to receive the copy, make sure to provide them with an authenticated delegation and a copy of your identity card. A member of our team will contact you for verification and validation before handing over any files.
