to be active members of your care
Bellevue strongly encourages family and friends to serve as active members of their loved ones’ care journey and participate in our Care Partner Program.
As a patient, you have the right to choose a family member or a friend as your care partner. The role allows the chosen member to assist the healthcare team by participating in the decision making and care plan.
The purpose of the Care Partner Program is to directly involve your family and friends in your hospital experience by giving them the opportunity to participate, access information and support you physically, psychologically, spiritually and emotionally.
The designation of a care partner also ensures that proper recovery and continuity of care is being implemented even after your discharge.

Additionally, your care partner can be involved in providing you direct care, such as helping with your medication, changing your dressing and assisting you with meals and bathing. They are encouraged to ask questions and raise concerns, participate in your bedside shift report and integrated care plan, and stay involved in your discharge planning.
