As a person-centered care hospital, we strongly encourage family and friends to be active members in the care of their loved ones through the participation in the “Care partner program.”
Any patient admitted to the hospital has the right to choose a family member or a friend as their “Care Partner.”
Any patient admitted to the hospital has the right to choose a family member or a friend as their “Care Partner.” The “Care Partner” can assist the healthcare team by participating in many ways and will be treated as a member of the healthcare team. The purpose of the “Care Partner Program” is to enhance the involvement of family and friends in the hospital experience by giving them the opportunity to participate in education and to provide physical, psychological, spiritual, and emotional support. Care partner participation ensures proper recovery and continuity of care after discharge. The care partner can be involved in providing direct care, such as helping with medications, changing dressings, assisting with meals, bathing, and other care activities. They can ask questions, participate in bedside shift reports and integrated care plans, and be involved in discharge planning.
