As a person-centered care hospital, we strongly encourage family and friends to be active members of your care and to participate in our “Care partner program.”

As a patient, you have the right to choose a family member or a friend as your “Care Partner.” The “Care Partner” can assist the healthcare team by participating in many ways and will be treated as a member of your healthcare team.

The purpose of the “Care Partner Program” is to enhance the involvement of your family and friends in your hospital experience by giving them the opportunity to participate in your education and to support you physically, psychologically, spiritually and emotionally.

Care partner participation ensures proper recovery and continuity of care after your discharge.

Your care partner can be involved in providing your direct care, such as helping with your medications, changing your dressing, assisting you with meals, bathing, and other care activities. They can ask questions, participate in your bedside shift report and integrated care plan, and be involved in your discharge planning.

Education during my stay

During your stay, we continuously provide you with verbal and written educational material that supports you and your family care partner in participating in care decisions and care processes.
